2 Form D - Notarised

4 Surety Bond in Form- E with PAN Card of the Surety and ITR
5 Solvency Certificate issued by …………………………..New Delhi for INR 5,00,000 (Five Lac Rupees)
6 Affidavit Regarding Regulation 5 .
7 TR 6 Challan Rs 5000 (Five Thousand) as license fee.
8 TR 6 Challan Rs 100 (Rs Hundred) as booklet fee.
9 PAN Card Copy of the applicant
10 Copy of Certificate of Incorporation ………………………….
11 Copy of result of CHA Examination under regulation 8 (Written & Oral) of CHALR2004
12 Character Certificate of Mr…………..
